In accord with US Catholic Bishops’ Charter for the Protection of Children and Young People (2002), all Religious Education Programs are required to implement Safe Environment Programs in the classroom.
In order to document how this requirement is met in religious education and youth ministry programs, we ask that you complete a Verification Report and return it to our office.
Two report forms are available – one for Grades K-8, another for high school, Grades 9-12. The forms are Word documents. Click on the download link below. The forms can be completed electronically or handwritten.
Please return the report(s), if at all possible, by May 21, 2010.

The forms ask whether you offered Safe Environment Education for children/youth during this past school year, 2008-2009, and what materials or presenters you used at each grade level. Some parishes use the lessons developed by the Archdiocese of Milwaukee and some use other resources.
These reports will be used 1) in the upcoming archdiocesan audit of Charter compliance, and 2) to help the archdiocesan offices assess the need for additional resources.
