Rite of Election - Sign Up

Please complete the Sign Up by February 4, 2012.

You must have all your names when registering. Do not use one name for several registrations, the name will not be accepted.

NOTE: This event is NOT for high school confirmation programs.

Please complete the sign up information for your contact person first, we need only the contact person's email. Be sure to print this page out before you submit it.

You will then need to fill out the form for each person, print the form and click on "Submit". If you are registering more than one participant, you will be directed back to the sign up screen to enter the next person.

Note: there will be a slight pause after submitting each name, wait for the confirmation message before proceeding.

Guests are welcome but we ask you not to include them. Very limited seating is first come first serve for guests.

There are limited numbers of seats available for each celebration. Your seat location will be determined according to the date and time your information is received, please make a first, second and third choice. The contact person will be notified of your seating assignment in a subsequent mailing, along with further instructions.
