Health Care Associate

  • Accompany SDS members to scheduled and emergency medical appointments.
  • Consult with medical, health care and pastoral care providers on behalf of SDS members.
  • Meet weekly (or more often if needed) with SSR Care Team. Help prepare for SDS member’s temporary, respite or permanent stay at SSR.
  • Prepare SDS members for upcoming medical appointments. Transport and accompany SDS members to serve as their patient advocate at medical appointments. Interpret and explain information conveyed by medical providers. Review/reinforce information with SDS members as needed.
  • Monitor SDS members’ ongoing and acute care needs. Report changes in medical conditions to appropriate providers and obtain orders as needed.
  • Coordinate hospital admission and advocate for hospitalized SDS member. Facilitate communications between the hospital, SDS member and SSR Care Team.
  • Update SSR Leadership Team and Provincial Leadership Team regarding SDS member’s health care status.
  • Communicate with pharmacies regarding SDS member’s medication needs and questions.
  • Document medical visits and incidents, and summarize in writing all information exchanged during visits, follow-up appointments, etc.
  • Communicate with RN-HCC, SSR Caregivers and support staff regarding an SDS member’s change in diagnosis, medication, treatment and care.



  • Solid employment history, with at least 2 years in medical- related position preferred
  • Valid Driver’s License and safe driving record
  • Use of personal vehicle for on-call hours
  • Physical strength/agility to walk long halls; push wheelchairs safely on uneven surfaces; stand for extended periods of time; bend to provide help in/out of vehicle; and routinely lift 25 pounds
  • Understanding of human bodily functions
  • English reading, writing and comprehension skills to communicate effectively
  • Ability to understand medical terminology and explain in simple language
  • Computer skills to communicate details related to medical visits, medical orders, follow-up appointments and treatments.
  • Basic understanding of medical/health insurance language.

  • Desire to work as part of a team and confidence to perform duties with minimal supervision


Hourly wage for scheduled hours worked, plus standby pay for on-call hours
