Nurse

Responsibilities:

  • Prepare SDS Sisters for upcoming medical appointments. Transport and accompany SDS Sisters to serve as their patient advocate at medical appointments. Interpret and explain information conveyed by medical providers. Review/reinforce information with SDS Sisters as needed.
  • Coordinate hospital admission/discharge and advocate for hospitalized SDS Sister. Facilitate communications between the hospital, Sister and SSR Care Team.
  • Consult with medical, health care and pastoral care providers on behalf of Sisters.
  • Help prepare for SDS Sister’s temporary, respite or permanent stay at SSR.
  • Monitor SDS Sisters’ ongoing and acute care needs. Report changes in medical conditions to appropriate providers and obtain orders as needed. Communicate with the SSR Care Team regarding a SDS Sister’s change in diagnosis, medication, treatment and care.
  • Communicate with pharmacies regarding SDS Sister’s medication needs and questions.
  • Document medical visits and incidents, and summarize in writing all information exchanged during visits, follow-up appointments, etc.
  • Overall responsibility for the SSR Wellness Office including keeping it organized and running efficiently. This includes the timely and accurate updating of paper and electronic medical records, coordination of healthcare volunteers, and overseeing the medication cart.
  • Cross-train with Province Nurse to provide back-up and/or assume tasks as time allows.

Qualifications

Qualifications

  • LPN or RN with three years of healthcare experience.
  • Driver’s license and access to reliable transportation for call response.
  • Ability to understand medical terminology and explain in simple language
  • Computer skills to communicate details related to medical visits, medical orders, follow-up appointments and treatments. Familiar with electronic medical records.
  • Strong organizational skills and attention to detail
  • Compassion for people with physical and mental limitations
  • Desire to work as part of a team and confidence to perform duties with minimal supervision
  • English reading, writing and comprehension skills to communicate effectively.
